This spell does not affect creatures which are immune to poison.
The target takes poison damage.
Then, the target gets a health save to avoid being paralyzed.
Fail:
If the target fails their save, they are paralyzed by poison.
They get another save at the start of each of their turns to end this effect.
(Unlike saves against most other effects, saves against incapacity generally occur at the start of a turn.)
Creatures which get multiple actions may spend an action to get a save to end the paralyzed condition from this spell on themselves.
(This ability is at its most potent when used immediately after your target’s turn.)
